Here is what will happen when your loved one begins receiving in-home care through Journey Palliative and Hospice:
- We will deliver Durable Medical Equipment, set it up, inspect it and test it.
- Our team will bring all other necessary medications and supplies to the patient’s residence.
- We will create a schedule for visits from the patient’s [Home Health Aid], doctor, nurses and specialists, as well as other team members like our social worker, chaplain and volunteers.
The schedule for care is fully tailored to the individual, and can be adjusted as needed. We will provide care at home as much as possible, only transitioning to in-patient care when necessary. As soon as your loved one is stable, they will be able to return home again.
In-home care is a collaborative process in which the patient’s primary caregiver and family play a pivotal role.
That means that at every stage, you will be able to give us input on the support your loved one needs and how best we can provide it.
